Mandatory COVID-19 Self-Declaration Form
Bring a signed copy of the self-declaration form in the format given below:

Items to be carried in the exam hall during SSC exam
Candidates must compulsorily bring the following at the time of reporting for the examination.
- Admission Certificate (Print copy of admission card).
- Two copies of his/her latest color passport size photograph (3 cm x 3.5 cm).
- At least one valid photo identity proof in original with clear photograph (eligible photo identity proof such as passport, print out of Aadhaar card/e-Aadhaar, driving license, service ID card issued to employees by Central Government/State Government) / PSU, ID card issued by University/College/School, Voter ID Card, PAN Card, Ex-Serviceman Discharge Book issued by Ministry of Defence, any other photo identity proof issued by Central Government/State Government).
- In case the photo identity card does not contain the complete date of birth, the candidate must bring an additional original document containing the date of birth printed on the admission certificate (for example admit card/pass certificate/10th class mark sheet issued only by CBSE/ ICSE/State Board; Birth Certificate, Category Certificate, etc. issued only by Central Government/State Government/PSU). In case of mismatch in the date of birth mentioned in the admission certificate and photo ID proof/certificate brought in support of date of birth, the candidate will not be allowed to appear for the examination.
