1. Block your phone number by calling the customer care service of your mobile network operator. This will prevent thieves from initiating transactions using your SIM card or lost mobile phone. You may be asked to provide your personal details such as your full name, billing address, and last recharge or bill amount paid for verification before your request can be processed.
2. To block your phone number from your account, call your bank helpline and ask to disable the UPI service.
3. File an FIR about losing your mobile phone. This can be easily done online for residents of most states in India.

A FAQ page on the National Payments Corporation of India (NPCI) site (FAQ page) explains how to block a mobile number. This can then be helpful in case the mobile phone is lost. However, there is no workaround available on the site by which the user can block his UPI access.
